Abstract:

Background: We aimed to analyze the prognostic impact of mucinous and non-mucinous rectal adenocarcinoma with stage II and III rectal carcinoma treated with radical surgery plus (neo) adjuvant chemoradiotherapy and evaluate disease-free (DFS) and overall survival (OS). Method: We conducted this retrospective study on patients with pathologically proven stage II/III rectal carcinoma and treated in the Department of Clinical Oncology and Nuclear Medicine, Mansoura University Hospital between January 2008 and December 2013. We designed a clinical abstract sheet and reviewed all cases in terms of history, clinical assessment, investigations done on the patients, and pathological reports including all the details, and treatment modalities, namely neoadjuvant and adjuvant. Results: The median DFS for non-mucinous adenocarcinoma (NMC) was beyond 60 months, while that for mucinous adenocarcinoma (MA) was 24 months (P=0. 008). The median OS for NMC was beyond 60 months; whereas, the mean OS of MA was 25 months (P=0. 002). Therefore, the difference between both groups was statistically significant regarding DFS and OS. Pathological subtype was the only statistically significant independent predictor in the three-year DFS. However, pathological subtype and lymph-vascular invasion were statistically significant independent predictors in the three-year OS. Conclusion: Histological subtype was an independent prognostic factor for both DFS ans OS in patients with stages II and III rectal carcinoma.

Abstract:

Primary neoplastic lesions presenting with a mucocele of the Appendix are very rare and can be divided in to benign variant of mucinous adenomas or cyst adenomas, mucinous tumours of uncertain malignant potential or mucinous cyst adenocarcinomas. Most of these tumors are asymptomatic and are found incidentally or by symptoms of simple appendicitis. We describe an unusual presentation of pseudomyxoma peritonei (the major complication of neoplastic mucinous appendiceal tumor) with urologic symptoms.

Abstract:

Background: The tumor suppressor gene TP53 (alias p53) located on chromosome 17 is involved in various cancers. Case-control studies have shown that p53 codon 72 polymorphism modulates the prognosis and susceptibility to various malignancies. We undertook the present study to explore a possible association between mucinous and non-mucinous adenocarcinomas with different genotypes or alleles at codon 72 of TP53.Methods: The genotype distribution and allelic frequencies for p53 polymorphism was assessed in 46 and 134 specimens from patients with colorectal mucinous and non-mucinous adenocarcinomas, respectively, by using allele-specific PCR. Results: The PCR products were 177bp for proline allele and 141bp for arginine allele. In the mucinous samples, the genotype distribution for p53 polymorphism showed 63%, 23.9%, and 13.1% for the Arg/Arg, Arg/Pro, and Pro/Pro genotypes, respectively. In the non-mucinous specimens 32.1% of the cases were Arg/Arg, 48.5% Arg/Pro, and 19.4% pro/pro. A significant difference between the two types of adenocarcinomas for the Arg 72 Arg genotype compared with (grouped) Arg 72 Arg and Pro 72 Pro genotypes was noted [OR=3.61 (1.76-7.27), P<0.001]. The arginine allele was found more often in patients with mucinous adenocarcinoma [OR=1.85 (1.07-3.19), P<0.03]. A higher portion of Dukes stage C was noted in the mucinous specimens (P<0.02) and also mucinous specimens were seen more often at advanced TNM stages (P=0.01).Conclusion: The Arg/Arg genotype at p53 codon 72 is more prevalent in mucinous colorectal carcinoma and the arginine allele may contribute to mucinous carcinogenesis. The proline allele was associated with higher Duke's staging in nonmucinous adenocarcinoma.

Abstract:

Metastatic disease to the oral cavity is rare, representing only 1-8% of oral malignancies, and involvement of the mandibular ramus is even less prevalent. This report is a unique case of metastatic adenocarcinoma of the colorectal to the mandible. Clinical presentation can be quite variable, and most often a primary malignancy is already known. Jawbone metastasis are a sign of disseminated malignant neoplasms, with poor prognosis and usually an indication for palliative therapy. The requirement for arrival at an appropriate and prompt diagnosis is crucial for determining the most appropriate treatment regimens and improved outcomes. The oral metastatic lesion may be the first indication of an undiscovered distant primary tumor, making timely evaluation and treatment critical from an oncologic perspective. Here we report a known case of mucinous adenocarcinoma of colon who was referred due to a mandibular lesion.

Abstract:

We present a case of adenocarcinoma developing at the vesicocutaneous edge of a vesicostomy, 8 years after it was created, in a patient who had neurogenic bladder secondary to medullitis. The patient died 6 months after the start of surgery therapy. Although transitional and squamous cell carcinoma of a vesicostomy have been reported, to our knowledge, the presence of adenocarcinoma at the vesicostomy edge without bladder involvement has not been reported previously.

Abstract:

Mature teratoma is a common tumor that can undergo malignant transformation, either in ovarian or extragonadal sites. While adenocarcinoma superimposed on sacrococcygeal teratoma is rare, mucinous variants have been reported in only five cases. Here, we present a case of a young girl with disseminated metastasis of mucinous carcinoma, initially of unknown primary origin. Further investigation by a dedicated multidisciplinary team (MDT) revealed a focus of mucinous carcinoma (intestinal type) in a sacrococcygeal teratoma incompletely resected five years earlier. The patient is currently undergoing second-line chemotherapy after experiencing side-effects on the first-line regimen. Pathologists, gynecologic oncologists, and surgical oncologists should exercise caution when dealing with locally aggressive teratomas, thoroughly searching for malignant components and conducting short-term follow-up.

Abstract:

Most adnexal masses found in pregnancy are simple benign cysts less than 5 cm in diameter. On the other hand, the majority of persistent adnexal masses 5 cm or greater in diameter are mature teratomas. Malignant transformation occurs in 0. 2 to 2 percent of mature teratomas, and squamous cell carcinoma arising from ectoderm is the most common secondary neoplasm. The mucinous cystadenocarcinoma transformation is very rare. We presented a 38-year-old G2Ab1 woman at 32 weeks of gestation with a huge, rapid growth adnexal mass who came to our emergency department due to severe preeclampsia, stage 1 of IUGR, and borderline AFI. After 48 hours of expectant management, a cesarean section associated with unilateral salpingo-oophorectomy was done, and the histopathology of the cyst revealed mature teratoma that was focally involved with mucinous cystadenocarcinoma. The complete surgical staging was done, and due to the stage of disease (1C1), she received adjuvant chemotherapy, and she has remained disease-free in her last follow up after one year.

Abstract:

In this case report, a man presenting with a rare symptom of appendix cancer is dis-cussed. Hematuria, which was the key diagnostic feature in this patient, has never been re-ported as a presenting feature of appendix cancer which may explain why this patient has been experiencing this symptom for months and yet have not get the proper di-agnosis. As we discussed in the case report, although appendiceal cancer is a potentially life-threatening issue, it is frequently missed in patients. Therefore, we believe report-ing this key symptom in this journal would help physicians to make a more accurate diagnosis in this matter.

Abstract:

A 76-year-old man with a past history of smoking referred to the dermatology department for an erythematoviolaceous, painful, indurated nodular lesion which had appeared since two months ago (Figure 1A). The lesion was fixed to the adjacent tissues. It was located on the right buttock measuring 4×4×1 cm. He was being followed for the last two years for a consolidation in the superior lobe of the left lung detected on chest CT-scan. It was stable in the follow-up scans, and results of the bronchial fibroscopy in two occasions did not show any evidence of malignancy...
